Our facility is open, and we are actively following the guidelines from the Department of Health and Human Services (STATE OF UTAH) and Center for Disease Control (CDC). - Trouble Breathing (unrelated to pre existing health conditions)
- Fever or Chills
- Within the last 14 days have you:
- Traveled to or from a country with a level 3 health warning:
China, Iran, South Korea, Europe (Austria, Belgium, Czech Republic, Denmark, Estonia, Finland, France, Germany, Greece, Hungary, Iceland, Italy, Latvia, Liechtenstein, Lithuania, Luxembourg, Malta, Netherlands, Norway, Poland, Portugal, Slovakia, Slovenia, Spain, Sweden, Switzerland, Monaco, San Marino, Vatican City United Kingdom and Ireland: England, Scotland, Wales, Northern Ireland, Republic of Ireland) - Traveled to/from Washington State, California, or New York?
- Been in close contact with a person with a laboratory confirmed case of COVID-19?
- As a healthcare worker, taken care of confirmed COVID-19 patients?
How to Stay Healthy and Prevent the Spread of Any Infection:
- Wash your hands often with soap and warm water for at least 20 seconds, especially after going to the bathroom; before eating; and after blowing your nose, coughing, or sneezing.
- Use an alcohol-based hand sanitizer with at least 60 percent alcohol if soap and water are not readily available.
- Clean and disinfect frequently touched objects and surfaces using a regular household cleaning spray or wipe.
- Avoid close contact with people who are sick.
